The following forms are available for download from this site. All of the forms are in PDF format, which requires Adobe Reader for viewing and printing. Reader can be downloaded (for free) from the Adobe Website.   Note that if you are using a Acrobat version 6 or higher,  most of the forms are "interactive", meaning that you can complete them from within Adobe Reader and print with your information typed in.  

Pre-Session Forms - for May Parent meeting

• Emergency/Disaster Release Form, required at the May Parent Meeting (print three copies per child: bring 2 copies to May Parent Meeting, bring 1 copy the first day of session)

• Parent Responsibilities Reminder List, bring to the May Parent Meeting to record your work days and responsibilities (for your convenience) 
 

• Medicine Authorization Form, if needed, due the first day of the session
• Board of Directors Nomination Form, nominate people for Board openings in the coming year; due by the Open House Night

Acceptance Forms

• Acceptance Packet:  Once your children are accepted into the session, this is the set of forms that you need to complete and return to complete your registration.
